Red Book Authorization Form

I authorize the American Kennel Club to release the breeding history of the following dog to the Red Book Committee of the German Shepherd Dog Club of America.  I understand the information received from the AKC will be used to compile statistics that will be published in the 2022 Red Book as well as Future editions.
