The application form can be found on page 57 of the membership directory. You can copy it and use it several several times over

Requirements for application for a dogs membership in the 13 Club:

    a) Have passed it's 12th birthday at the time of application. Does not have to be living at the time of application.

    b) Requires the submission of a copy of the AKC/CKC registration.

    c) Copy of 4 generation pedigree.

    d) Completed application form.
