The 2008 Futurity Litter Nomination Form is available in multiple formats (each one can be downloaded after opening it by selecting "File  /  Save As"  ) :

• PDF format  -- for printing only   <== Recommended form to use

Nomination Fee is now $25.00
note applicable late fees listed on the form
NOTE 1:  It is recommended, regardless of which option you chose, to first download the selected file.  That way you always have it immediately available should you need another copy, or multiple copies.

 NOTE 2: After filling out the the form, mail it along with your $25 (plus any late fees -- as applicable), to GSDCA Futurity
