following official entry forms are to be used for this year. These are all
PDF types.
Click on the form you need.
Once opened do "File / Save As " to download the form to your computer.
Once the file is in your computer you can open it again and then print as many
copies as you need.
NOTE: Each form consists of 2
pages. Make sure you fill out and submit both pages.

You can fax Rau at (610-376-4939)(extra $4.00 charge). Make sure
you keep proof
of when you faxed in case there is a mix-up.
You can FedEx - Keep your
receipt.
Regular mail - mail early as you never know these days. You can
call and verify that it got there if you want.
